Workmen's Compensation / Checklist
 

Kindly submit the claim form together with the following supporting documents :-

  • Photocopy of BUR555/BUR500
  • Photocopy of Identity Card/ Passport of injured worker
  • Photocopy of Form A (to Labour Department)
  • Original Medical Bills and Medical Report
  • Every Letter, Claim, Writ, Summons and legal proceedings should be notified or forwarded to National Insurance Company Berhad immediately on receipt.
