Kindly submit the claim form together with the following supporting documents :-

• Accident report form duly completed and signed by the Insured and Driver
• Police report 252
• Police Statement 
• Photocopy of vehicle registration card
• Photocopy of Driving Licence and Identity card for both owner and driver
• Photocopy of customs vehicle pass
• To bring along Original Driving License and Identity Card for verification.
• Every Letter, claim, Writ, Summons and legal proceedings should be notified or forwarded to National Insurance Company Berhad immediately on receipt.
• As long as your vehicle is insured with National Insurance Company Berhad, you are required to report the accident to the Company immediately.
