Kindly submit the claim form together with the following supporting documents :-

  • Police Report
  • Notice of claim from third party or their legal representatives
  • Quotations on replacement or repairs to damaged property
  • Any other documents or information pertaining to the claim
  • Every Letter, Claim, Writ, Summons and legal proceedings should be notified or forwarded to National Insurance immediately on receipt
